Contemporary Dental Implant Center and all affiliated locations are permanently closed.
Timeline of Operations
March 30, 2020
Selected locations ceased operations at the onset of the COVID-19 national pandemic.
April 1, 2023
Notice provided that all remaining locations would permanently close.
December 31, 2023
All remaining facilities and clinical operations concluded.

The following locations permanently ceased operations on or before March 30, 2020:

- 50 Broadway, Suite 2001 New York, NY 10004
- 601 Franklin Ave, Garden City, NY 11530
- 97-30 64th Ave, Rego Park, NY 11374
These locations have not been operational since that time.

Following notice issued April 1, 2023, the remaining locations permanently ceased operations as of December 31, 2023:

All Contemporary Dental physical facilities and corporate clinical operations are permanently closed.
Records Retention
Patient treatment records are retained in accordance with New York State Education Department (NYSED) recordkeeping requirements.
Records are maintained for a minimum of six (6) years from the date of last treatment.
Records remain accessible pursuant to these requirements.
Independent Treating Dentist Notice
During April 2020 – December 2023, treating dentists provided services as independent licensed providers.
The former corporate entity is closed permanently, does not provide clinical care and functions solely as an intake point for records requests.
For accurate retrieval and continuity matters, patients must identify their independent treating dentist when submitting a request.

Records Custodian
Patient records are maintained in accordance with New York State requirements and are managed by an authorized records custodian for retention and release purposes.
Records requests submitted via the email address above will be forwarded to and processed by the authorized records custodian.
